The survey, which is voluntary and takes 20 to 25 minutes to complete, is anonymous.

It includes questions such as which COVID-19 related services were accessed, which government agency or representative is believed to have had the strongest influence on decision making, and where COVID-19 information was accessed.

The survey also includes questions about how WSDOH addressed misinformation and vaccine hesitancy, whether information was provided in one’s spoken language, and whether COVID-19 resources were equitably distributed throughout the state. Some of the questions have fill-in-the-blank answers while others have a selection of answers to choose from.
