Singapore Adds HPV9 Vaccine to National Immunisation Schedules from Jan 1, 2027

Singapore will include the HPV9 vaccine in its national immunisation schedules from Jan 1, 2027, offering broader protection against cervical cancer to females aged 9 to 26, with new subsidies and MediSave coverage.

Singaporean and permanent resident females aged 9 to 26 will have access to broader protection against cervical cancer from Jan 1, 2027, when the HPV9 vaccine is added to the national immunisation schedules.

HPV9 Schedules and Vaccination Choices

The Ministry of Health, Communicable Diseases Agency and Health Promotion Board announced the change in a joint statement on Friday, Oct 2. The HPV9 vaccine, also known as Gardasil 9, will be included in the National Childhood Immunisation Schedule, the National Adult Immunisation Schedule and the Subsidised Vaccine List. The current HPV2 vaccine, also known as Cervarix, is included in both schedules and will remain available as an alternative option.

Both vaccines protect against HPV types 16 and 18, which cause about 70 per cent of all cervical cancers. The HPV9 vaccine provides broader protection by also covering additional HPV types that account for a further 20 per cent of cervical cancers. This brings its total coverage to approximately 90 per cent. Under updated recommendations by the Expert Committee on Immunisation, unvaccinated females may start or complete their vaccination series with either HPV2 or HPV9.

Two doses are recommended for those receiving their first dose from age 9 to 14. Three doses are recommended for those receiving their first dose from age 15 to 26. If HPV vaccination is started but not completed by age 26, the remaining doses may be taken after age 26, as early as possible and up to age 45. Females who started their series with HPV2 or HPV4 and have remaining doses may complete the series with HPV9.

HPV4, previously known as Gardasil, is no longer available in Singapore and has been removed from the NCIS and NAIS. Studies show mixed HPV vaccination schedules are safe and provide protection against the HPV strains covered by both vaccines. Those who have completed their recommended schedule do not need additional doses and are not advised to receive extra HPV9 doses.

HPV Risks and School Vaccination

HPV is the most common sexually transmitted infection worldwide, with nearly 80 per cent of men and women infected at some point in their lives. It can cause cervical cancer, as well as cancers of the anus, throat, vulva, vagina and penis. Under the Health Promotion Board school-based HPV vaccination programme, Secondary 1 and 2 female students are offered vaccination at no charge.

From Jan 1, 2027, HPV9 will replace HPV2 for female students who have yet to start or complete their series under the programme. Female students who received HPV2 as their first dose in 2026, including the 2026 Secondary 1 cohort, will receive HPV9 as their second dose in 2027. Female students who have already completed their HPV vaccination series are not eligible for further HPV9 doses.

From Jan 1, 2027, eligible Singaporean children will receive full subsidies for HPV9 vaccination at CHAS GP clinics and polyclinics. Eligible Singaporean adults can receive means-tested subsidies of up to 75 per cent at these settings. Eligible permanent residents can receive subsidies at polyclinics. MediSave use will be extended to HPV9 vaccination for people in the recommended groups from Jan 1, 2027.

Subsidies and MediSave Support

This is subject to prevailing MediSave Chronic and Preventive Care withdrawal limits. Eligible groups can use up to $700 of their MediSave Chronic and Preventive Care limits for the HPV9 vaccine. The limit increases to $1,000 for patients treated for at least two Chronic Disease Management Programme conditions within a calendar year. It also increases for one CDMP condition with complications.

Vaccination subsidies and MediSave use will not apply to additional doses of HPV9 for those who have already completed the recommended doses. Government subsidies and MediSave are currently not available for HPV9 vaccination. At Singapore Women's Clinic, HPV9 currently costs more than twice as much as HPV2, at S$250 per dose compared with S$96 per dose. According to the MOH vaccination website, HPV2 is registered for use in females, while HPV9 is registered for both females and males.

Eligible individuals can book HPV vaccination appointments at participating CHAS GP clinics through vaccine.gov.sg or the national vaccination booking portal. They can also make an appointment at a polyclinic. HPV9 vaccination under the updated schedules will be available from Jan 1, 2027.
